How to View Facility Health Care Safety Data Report

  1. Find a facility using the steps in Health Care Safety Data Home - Facility Search or Map Search.
  2. On the Facility Search Results page, click on the link that says "Select" next to the facility whose Health Care Safety Data data reports you would like to view.

This will open the most recent Consumer Health Care Safety Data Report for that facility in a new browser window or browser tab. The Facility Search Results window will stay open.

NOTE: Your browser’s pop-up blocker must be disabled and Javascript must be enabled. For instructions on how to disable your browser’s pop-up blocker or to enable Javascript, go to Frequently Asked Questions in the Main Menu.

VIEW SAMPLE FACILITY HEALTH CARE SAFETY CONSUMER REPORT: (Sample Consumer Report)

  1. To view the facility’s Technical Health Care Safety Consumer Report, go back to the Facility Search Results window.
  2. Toward the top of the page, find the Technical Reports header. Click on the link with the date range specified. This will give you the report for the time period selected.
  3. Next to the Technical Reports header, Click on the Sample link which will provide a description to each type of item identified in the report.

The Facility Search Results window will stay open. For more information about the Consumer and Technical versions of the HAI Data, click on Understanding the Data in the Main Menu.

Other information about Health Care Safety Data Reports:

More than one Health Care Safety Data Report can be open at the same time in different browser windows. Reports can be printed using the browser Print function (Ctrl+P).

Not every facility will have a Health Care Safety Data Report. Some facilities are not required to report Health Care Safety data to the Texas Department of State Health Services (DSHS), according to Chapter 98 of the Texas Health and Safety Code. These facilities will have a report with a message that says,

This facility is not required to report data to the Texas Department of State Health Services for this reporting time period.

Facilities that were required to report Health Care Safety data to DSHS but failed to do so for the reporting time period, will have reports with a message that says,

This facility failed to report data to the Texas Department of State Health Services for this reporting time period.
