HOW TO SEARCH FOR A FACILITY USING FACILITY NAME:

  1. Find the field called Facility Name. Enter part or all of a health care facility name here.

    NOTE: if you are unsure of a facility's exact name, enter only part of the name and choose Name contains this text. Click the Search button.

  1. If search by county is desired, there is a Clickable link for "Search by Map" that invokes a Texas Counties Map that provides quick selection of county with facilities.

  1. After clicking Search, a set of results will appear for the Facility Name that was entered.
    2. In our example, we entered "Scott" in the Facility Name field. Here is a list of facilities containing "Scott" in their name:
